Instructions for Preseason Packet—Returning Students

Enclosed in this packet you will find the medical paperwork you need to complete for your continued participation in athletics. Please fill out all information to the best of your knowledge, and make sure you fill out each page completely without any blanks.  This packet should be completed and returned by August 1.  You should drop off or mail this completed packet back to:

Each athlete at Columbia College must have a physical on file before participating in any practices, conditioning, or games.  Physicals are provided on campus free of charge by our team physicians at the Moore Clinic.  The dates for physicals are Tuesday, August 9 at 5:30 pm for soccer and volleyball athletes and will be announced for softball, basketball and tennis athletes.  Please make every effort to be present at these times, as we will not accept physicals from any other physician.

On our website you will also find the Medical Policies and Drug/Alcohol policy that the athletic department follows.  There is also a statement on secondary insurance claims that you will want to read through and/or give to your parents.  The Drug/Alcohol Policy also has an acknowledgement/consent form that you need to sign.

In addition, the College is continuing the hard-waiver form for medical insurance.  Your tuition account will be billed for medical insurance through the school policy with Pearce and Pearce.  If you already have medical insurance through your parents or another policy, you MUST go online to the Pearce and Pearce website to waive this insurance and refund these charges.  This waiver should be ready by the end of July, and I recommend you complete your waiver before arriving on campus so that it is processed in a timely manner.  If you are covered under your parents plan, I would recommend that you keep your current insurance instead of purchasing the school plan because this plan is very basic and does not cover athletic injuries.  You can find the waiver link at:
