Instructions for Preseason Packet—New Students and Transfers
Dear Student-Athlete,
Hello! We would like to take the time to welcome you to Columbia College athletics. We look forward to meeting each of you as the school year begins.
Each athlete at Columbia College must have a physical on file before participating in any practices, conditioning, or games. Physicals are provided to student-athletes prior to the start of their season. The dates for fall physicals are Tuesday, August 9 at 5:30 pm for soccer and volleyball athletes and will be announced for softball, basketball and tennis athletes. Please make every effort to be present at these times, as we will not accept physicals from any other physician.
Enclosed in this packet you will find the medical paperwork you need to complete. Please fill out all information to the best of your knowledge, and make sure you fill out each page completely without any blanks. This packet should be completed and returned by August 1. On our website you will also find the Medical Policies, Drug/Alcohol Policy, and Insurance Procedures that the athletic department follows. Please familiarize yourself with these policies and retain them for future reference. These packets also have acknowledgement/consent forms at the end that you need to sign and turn in.
In addition, the College is continuing a hard-waiver form for medical insurance. Your tuition account will be billed for medical insurance through the school policy with Pearce and Pearce. If you already have medical insurance through your parents or another policy, you MUST go online to the Pearce and Pearce website to waive this insurance and refund these charges. This website should be ready by the end of July, and I recommend you complete your waiver before arriving on campus so that it is processed in a timely manner. If you are covered under your parents plan, I would recommend that you keep your current insurance instead of purchasing the school plan because this plan is very basic and does not cover athletic injuries.
Please note that each athlete must have ALL medical paperwork on file and pass their physical before participating in any practices, conditioning, or games:

- Secondary Insurance Authorization
- Copy of the front and back of your insurance card
- Summit America Secondary Insurance form
- Assumption of Risk form
- Consent/Disclosure of Health Information form
- Columbia College Consent and Release
- Providence/Moore Consent and Release
- Providence/Moore Authorization to Disclose
- Clearance Form (if you have been injured since your last physical or if you are receiving treatment for a current injury, please bring a letter from your physician stating your participation status)
